A.S.A.P. HEALTH AND HUMAN SERVICES, INC. is dedicated to uplifting Florida's vulnerable communities, including the low-income, elderly, and those with disabilities. We provide essential programs promoting self-sufficiency and improved quality of life.
At ASAP Health and Human Services, our initiatives are designed to support underserved individuals and families who face barriers to care, including lack of insurance, homelessness, financial hardship, mental health challenges, domestic violence, substance abuse, and limited access to community resources. Through compassionate, community-based programs, we strive to promote stability, wellness, independence, and hope for vulnerable populations.

Health Initiative

Homeless Outreach Initiative
Providing access to mental health support, counseling referrals, emotional wellness education, and crisis intervention services for uninsured and low-income individuals experiencing depression, anxiety, trauma, or emotional distress.
​Supporting individuals and families facing homelessness or housing insecurity by connecting them with shelters, transitional housing resources, rental assistance programs, and community support services.
Food & Basic Needs Assistance Initiative
Helping families access essential resources including food distributions, hygiene products, clothing assistance, school supplies, and emergency household necessities through partnerships and community outreach efforts.

In collaboration with Avila Health Group's expert case management team, ASAP Health & Human Services advocates on behalf of clients to ensure access to essential services and supports. This includes assisting individuals and families with navigating complex healthcare and social service systems, connecting them to community resources, coordinating care among multiple providers, facilitating referrals, and empowering clients to achieve their personal, health, and recovery goals. Through this integrated approach, clients receive the guidance, support, and resources necessary to improve their quality of life and achieve sustainable outcomes.

Case Management and Care Coordination Services:
Through its partnership with Avila Health Group, ASAP Health & Human Services provides comprehensive case management and care coordination services designed to address the unique needs of individuals and families.
Working alongside Avila Health Group’s experienced team of case managers and behavioral health professionals, services include comprehensive needs assessments, individualized service planning, resource linkage and referrals, healthcare navigation, benefits assistance, advocacy, crisis intervention, and ongoing monitoring of client progress.
Community Partnerships and Resource Coordination:
ASAP Health & Human Services collaborates with healthcare providers, behavioral health organizations, nonprofit agencies, schools, faith-based institutions, and community partners to enhance service delivery and improve outcomes for individuals and families.
Through partnerships with organizations such as Avila Health Group for mental health and substance abuse services and Bridge to Hope for food assistance, we help connect those in need to critical resources, promote self-sufficiency, and strengthen community support networks.
Immigration Assistance Initiative
Supporting immigrant individuals and families as they work toward stability, legal status, and citizenship. Many immigrants face barriers such as language difficulties, limited resources, lack of insurance, unfamiliarity with community systems, and difficulty accessing services. Our initiative focuses on providing guidance, support, education, and case management to help individuals successfully navigate these challenges.
DCF Access Site:
As an ACCESS Community Partner Site, ASAP Health & Human Services provides free assistance with government benefit applications, resource navigation, advocacy, care coordination, and referrals to healthcare, behavioral health, food assistance, and other community-based support services.
